1993 First Release! The formal release was made in a clamshell. Somewhere around 2-10% of copies were made with a slip cover—for rental stores and non standard distribution batches. What that means, for instance, the lowest priced listings of Disney Pixar’s “Cars” on eBay, are for upwards of $800. Meanwhile, there are no known available copies of Free Willy 1 in a slip online. This would be the first I’ve seen, myself.
