This is the Koga’s Grimer card from the Gym Challenge set, a Basic Grass-type Pokémon with 50 HP. It has two attacks: Sludge Grip (one Grass energy), which on a coin flip lets you swap in one of the opponent’s Benched Pokémon as the new Defending Pokémon and Poison it, and Sludge Toss (one Grass and one Colorless energy), which deals 20 damage. The card shows a weakness to Psychic and a retreat cost of one energy, with Koga’s signature “trainer” artwork insignia in the bottom right, and is common rarity.
